Most of my readers are pretty savvy about nutrition. They stay on top of the latest developments and think carefully about their food choices. But over the years, I've noticed a few traps that even health-conscious people often fall into. One of the most common mistakes is to assume that foods are good for you simply because they're all-natural.

I think it's especially easy to fall into this trap when shopping at health or natural food stores. Everything you see is the healthiest possible version of itself. The sodas are sweetened with organic cane sugar instead of high fructose corn syrup. The potato chips are made with olive oil and air-dried Celtic sea salt. The ice cream is made from rBST-free cream. The jelly beans are colored with all-natural dyes. That's all very well and good, but at the end of the day you still have a cart full of soda, potato chips, ice cream and jelly beans.

It's just as easy -- and damaging -- to over-consume natural sugar as it is to overdo it with refined sugar. And frying a potato in olive oil and sprinkling it with sea salt doesn't turn it into a health food any more than coloring a jelly bean with beet extract turns it into a nutritious choice. I know that sounds ridiculously obvious. But you would be amazed how easy it is -- even for nutrition-savvy people -- to get distracted by the so-called "health halo."

Keep Your Eye on the Rabbit

Just like the magician who does something eye-catching with his right hand so that you don't notice what he's doing with his left, marketers can often get you to forget what you know about nutrition by focusing your attention elsewhere.

If I asked you whether organic foods are lower in calories than non-organic foods, for example, most of you would answer -- correctly -- that being organic has absolutely no bearing on a foods' caloric value. But if I showed you a plate of food, described the preparation and asked you to estimate how many calories it contained, you'll probably give me a lower estimate if I include the word "organic" or "natural" in my description.

This phenomenon has been documented over and over again. A couple of years ago, John Tierney of The New York Times found that people estimated the calories for a meal as being 200 calories lower when it was accompanied by a packet of crackers labeled "trans-fat free." More recently, a researcher from Cornell University asked people to compare organic and conventional versions of the same cookie. In reality, the cookies were identical but people estimated that the faux-organic brand was lower in fat and higher in fiber. Even so, people rated them as better-tasting!

Be On Guard Against the Health Halo

There are a lot of good reasons to choose foods that are less-processed and more natural. You can reduce your exposure to pesticides by choosing organic foods, for example. Just don't fall into the trap of assuming that a food is more nutritious (or less damaging) just because it's natural. And don't let food marketers get away with the old magician's distraction trick. Hold organic and all-natural products to the same standard you'd apply to conventional goods. Check the nutrition facts label to see how the calories, fat, sodium and sugars add up. Organic or not, make sure that fresh fruits, vegetables and other whole or minimally-processed foods are taking up most of the space in your cart and that chips, crackers, cookies and other treats are in the minority.

Shoveling Snow? How to Protect Your Back (And Your Heart)

Snow and ice can make driving treacherous, of course, but snowfall -- especially the wet, heavy kind -- can be dangerous even if you never leave your driveway. Each year, shoveling piles of snow after a storm is believed to cause tens of thousands of back and shoulder injuries in the United States, not to mention several hundred heart attacks.

Overall, more than 70,000 people ended up with a shoveling-related injury bad enough to trigger a doctor's visit in 2008, according to the Consumer Products Safety Commission. A quarter of those people visited an emergency room, and about 900 were admitted to a hospital.

The exertion, cold weather and slippery surfaces snow shovelers face are a dangerous combination, especially if it's an activity you're not used to. Snow shoveling "is one of the most high-intensity exercises you can do," says Suzanne Steinbaum, D.O., a preventive cardiologist at Lenox Hill Hospital, in New York City. "You're using all your major muscle groups."

But there are steps you can take to shovel safely and ensure that you survive the winter in one piece.

Back Pain
The number-one injury sustained after a snowstorm is lower back strain, according to Henry Goitz, M.D., an orthopedic surgeon at Henry Ford Hospital, in Detroit, and a spokesman for the American Academy of Orthopaedic Surgeons (AAOS). "That's when a muscle gets over-tensioned and tightens," he explains. "If it over-tightens, it's almost like a spasm and that gets very painful."

Another common back injury incurred during shoveling is a herniated disk, says Victor Khabie, M.D., the co-chief of orthopedic surgery at Northern Westchester Hospital, in Mt. Kisco, N.Y. A herniated disk (also known as a slipped disk) is when one of the soft disks between the vertebrae comes out of position and pushes on a nerve.

If your post-shoveling recovery includes back pain that radiates down your leg, it's a bad sign. "That may mean you have ruptured or herniated a disk," Dr. Khabie says.

The combination of frigid weather and normally sedentary people going all-out can be a recipe for back injuries. "The shoveling tends to be done by people who are not otherwise in good shape," says Richard Pomerantz, M.D., a professor of medicine in cardiology at the University of Rochester Medical Center, in Rochester, N.Y. "Sometimes a potato chip is the heaviest thing they've lifted for a while."

The snow is not the only danger. Ice lurking underneath snow can potentially cause even worse injuries. Evalina L. Burger, M.D., a vice chair and associate professor of orthopedics at the University of Colorado, in Denver, says she has seen no acute injuries from shoveling over the past three years but "horrible injuries from slipping on ice, including bad fractures."

"The worst thing is a fall," she says. "It's not just old people who fall."

Heart Attacks
Though rare, heart attacks that occur while shoveling snow have a long-standing place in medical lore -- much like heart attacks that occur during sex.

In the late 1970s, researchers from the Centers for Disease Control and Prevention studied six major snowstorms in Massachusetts (including the record-setting Blizzard of '78), and found that heart disease-related deaths rose by 22 percent in the week following a storm. Men were responsible for the majority of the spike, which "might be explained by snow shoveling," the researchers noted.

The combination of exertion and cold weather are believed to be the cause of shoveling-related heart attacks. "We don't like heart patients doing tons of heavy weight lifting coupled with cold weather," says Dr. Pomerantz, whose home of Rochester averages about 7 feet 8 inches of snow each year.

There's both an aerobic and weight lifting component to snow shoveling, says Dr. Pomerantz, and the weight lifting portion can raise blood pressure. "You're increasing the load on the heart very, very quickly when maybe the heart is not used to it," he says. "If you do that in a setting where it's cold, your arteries and vessels tend to constrict, so the relative blood supply goes down at a time when you're asking the heart to do a lot more. Demand is high and supply is low and that can be too much sometimes."

Moreover, hormones released during cold weather and exercise can cause plaques -- fatty deposits that line artery walls -- to rupture, leading to blood clots and heart attacks. "If you have plaque in your arteries, this is a perfect time for them to rupture and lead to a heart attack," Dr. Steinbaum says.

Timing may also be a factor. According to a 1996 study in the American Journal of Cardiology, many people shovel in the early morning, when the risk for heart attacks is higher.

Shovelers should follow a few simple rules:

  • Stay in shape. People who are in shape year-round have less to worry about when the flakes start falling.
  • Work up to it. Warm up inside, and incorporate stretches (especially for the hamstrings) as well as core-strengthening exercises such as mini sit-ups, crunches or working an exercise ball.
  • Don't try to do it all at once. Shovel small amounts of snow at a time, especially if the snow is heavy and water-laden. "If you're not used to exercising or you've had a back problem in the past, the smart way of approaching this is to do a little bit at a time," says Dr. Khabie. Shovel for five or 10 minutes, then go back inside for 10 minutes to do more stretching.
  • Practice proper technique. The AAOS advises pushing the snow-containing shovel instead of lifting it. But if you do have to lift, "do more of a knee squat, your back arched as if looking to the sky," Dr. Goitz advises. Don't bend from your waist. When disposing of the snow, keep the shovel close to your body and, again, squat with your legs, Dr. Goitz says. Walk over to where you're going to dump the snow, as opposed to leaning over and throwing it. By no means should you rotate or twist your body. "That's the worst thing you can do for your back in general," says Dr. Burger. "That's how you herniate disks."
  • Pick the right tools. Ergonomic shovels "tend to take the stress off the back," Dr. Khabie says. Adds Dr. Burger, "The bent handle gives you a better angle. You don't have to go down so far and you don't have to go all the way back up."
  • Layer with warm clothing. Being underdressed will leave you vulnerable to a painful strain. "You don't want to go out there and really freeze because it will tighten up your muscles," Dr. Khabie says.
  • Wear snow boots with good traction. "If your feet slip, it throws off your entire balance," Dr. Khabie says. "Find a good, sturdy snow boot that's made to be out in the snow." Consider keeping a set of spikes in your car, Dr. Burger adds. "They take two minutes to put on -- versus six weeks off from work if you injure yourself."

The cardinal rule is to practice a bit of common sense. As with any exercise, if you're out of shape, have had back injuries in the past, or have a history of heart disease, check with your doctor before tackling that snowdrift. This is wise even if you don't have heart problems but have high blood pressure or diabetes, or are overweight -- all of which are risk factors for heart disease.

For some people, the safest thing to do might be to outsource the job to a son, daughter or neighborhood teenager who's looking to earn a little pocket money.

I run a small home-based business that has very flexible hours. How many of you can relate to this? You are out walking (or playing a casual game of basketball) when all of a sudden -- pow!! Your ankle gives out and it collapses under your body weight. You hit the ground hard. People gather around you telling you to get up and walk it off. Others are telling you to put ice on it. Others examine it with their unprofessional gaze and then tell you not to worry. If you can walk on it, it's not broken. So you manage to limp home and get off your foot. A couple of days later, the ankle has become very swollen and painful. It's all "black and blue." So you go to the emergency room or to one of the "quick care" centers to have it checked. An X-ray is taken. You are told it's not fractured and not to worry, it is only a sprain. They put an Ace bandage on it. No other care is rendered. And you spend the next couple of months dealing with swelling, pain, stiffness, muscle spasms and secondary pains from compensating for the ankle sprain. You tape it, you tie it, you wrap it, you soak it. And finally, it "heals." Or has it?

Somewhere down the road, it happens again, and maybe again and perhaps again after that. You catch my drift here. This is an ankle that has never properly healed and is left with weakened ligamentous support, making you vulnerable to chronic ankle sprains and a life of chronic pain.

So why would something like a simple ankle sprain wind up creating chronic ankle instability? To understand this, let's look at the actual dynamics here. An ankle sprain can lead to an over-stretched ligament that can cause bleeding in the "pulled-open" spaces of the tissue, it can cause a partial tear of the ligament, it can cause an avulsion (where the ligament is pulled off of its bony attachment) and of course, a complete tear of the ligament. Since there are numerous ligaments supporting the ankle on both sides, depending on the type of forces that have occurred, there can be multiple ligaments involved in the injury. The mistaken notion that if it's not broken, it does not require treatment could not be further from the truth.

Ligaments are extremely condensed tissues. They have no direct arterial blood flow. They receive oxygen and nutrients from microscopic circulation. So in the best of circumstances, blood flow to ligaments is somewhat restricted. Since the healing of all injuries depends on good perfusion of the involved tissues, ligaments are at a distinct disadvantage due to their inherently poor blood supply. If you put this together with some other factors that I will enumerate, inadequate treatment may result in painful, chronic ankle instability.

So what are the other factors affecting healing in a case such as this? Most important would be compression and immobilization. Using an Ace bandage may limit some swelling, but it does not immobilize the ankle. Movement and the stress of weight-bearing on these tissues makes it that much more difficult for the ligament to heal. In most cases, I will use a removable walking brace which functions as a cast but can be removed for sleeping, bathing and therapeutic treatments. As I explained, since ligaments are poorly perfused with blood, therapy designed to increase blood flow into the microscopic circulation is very important. In my office, I use MicroVas® therapy, which is a bilateral high voltage stimulation treatment which dramatically increases blood flow to these injured tissues. It decreases healing time which also helps reduce the length of time the patient is forced to compensate and overuse the other limb, often leading to secondary pain syndromes. Additionally, there are ways to assist the body in re-organizing and healing the injured tissues. I have been using a treatment known as prolotherapy for many years, which involves the injection of low level irritants into the area of the injured ligaments. The irritation leads to a more profound push by the immune system to heal these ligaments by fostering the migration of fibroblasts (which are the cells responsible for healing connective tissue) thereby increasing the collagen matrix. Prolotherapy also increases the rate of growth of new blood vessels to the injured tissues (known as angiogenesis) which further facilitates healing. When prolotherapy is not successful, we can improve outcome by using platelet-rich plasma injections. This is the injection of the patient's very own platelets into the area of injury. Platelets are very high in growth factors which can be helpful with many difficult and stubborn cases.

Lastly, making sure that the patient is properly hydrated improves peripheral blood flow and assists in the delivery of nutrients and oxygen and the removal of inflammatory infiltrates from the area. This means drinking plenty of water and limiting dehydrating compounds such as caffeine, alcohol and salt. I have also found an improvement in healing my patients with the use of proteolytic enzymes (which also help break down inflammatory infiltrates and catalyze repair processes) as well as dietary sulfur, known as MSM, which is the bond/framework of connective tissue.

Treating the majority of ankle sprains in this fashion will dramatically improve outcome and prevent an enormous number of chronic ankle instability. However, once the condition is set as a chronically unstable ankle, many of these patients go on to ankle stabilization surgeries. My experience shows that a conservative attempt at healing this condition in this way is always worth the effort, as many of my patients avoided the necessity of surgical intervention.

If I were a camel, I would have surely buckled under that slender straw, which took the form of a nail file. Searching my bedside table drawer for the six-inch metal instrument, rifling through paraphernalia such as dog-eared business cards, highlighters of every hue, numerous white packets stamped "extra buttons," crumpled to-do lists, nonworking wristwatches, orphan earrings and a green tea scented candle in a tin, I failed to find the tool.

Then the straw: stirring up the scads of expendables I should have tossed years ago made the drawer impossible to shut.

"Away with it all now," I thought, not just for myself, but for whoever would have to clean up after me should I die suddenly by accident, or slowly from the cancer that may still lurk within. Last week's CT scan had shown that one of the small "ditzels" in my lung seems to have grown a bit; if I needed more treatment, who knew if I'd have the time or energy for spring cleaning?

I resolved to trash, recycle or donate everything but the barest necessities (thankfully, I didn't find anything in the drawer to compost!), and commenced my winter cleaning-sorting spree. Along the way I learned, or re-learned, several lessons many people conveniently forget because they think that they have all the time in the world. Not so for those of us punched by cancer.

Cancer. It teaches so much, even as it confounds and confuses. James Dean, whose young life was snatched not by cancer but a car crash, wisely advised, "Dream as if you'll live forever, live as if you'll die today." But where does that put you in terms of retirement savings or long-term care insurance? To splurge or not to splurge? That is my question.

There really is no absolute truth for people in my -- or any -- situation. Achieving balance is a life's work. But one thing became incontrovertibly clear as I stared down at the junkyard of memories: I had to take advantage of this opportunity to gain control by letting go, to defy life's contradictions by holding them all true, and to solve life's mysteries by embracing their insolvability.

Heretofore is what I re-learned from $#!*$%! cancer and the drawer that stayed stubbornly ajar until I rended it from its runner and gutted it clean.

1. Let go/Hold on

• Let go of unessential objects. If something takes up physical space, it likely crowds your psyche too. You deserve room to breathe, create and discover. (You may discover buried treasures as well as nail files in the process.)

• Purge yourself of impossible expectations and unattainable goals. Accept that you will never learn Swahili or meet Sting; stop expecting to hear gentle words of love from those who don't know how to speak them. As the saying goes, "The definition of insanity is doing the same thing over and over again and expecting different results."

• Let go of people who don't fulfill, but consistently disappoint you. Just because you've had a relationship, however unsatisfying, for decades doesn't mean you must continue it.

• Hold onto and nurture relationships that do feed you. Make time for people who understand you, who keep you nimble with challenges but also safe with tenderness, who make you laugh and let you cry.

• Hold onto someone, preferably someone you like. An embrace or hug releases the hormone oxcytocin, which stimulates feelings of love and quells anxiety.

• Hold fast to the seven items you'd grab if your house were ablaze. My list includes my son's Femo sculpture of a breakfast-in-bed tray, a treasure I found in my untenable drawer.

• Hoard chocolates. If you buy a box of handmade truffles for a friend but know you secretly bought it for yourself, admit it and keep it as your private stash. Better yet, buy two boxes, one for your cherished and one for yourself.

2. Finish what you start/Just walk away

• Finish that letter to your daughter that you started, just in case something should happen to you. Put the final punctuation on that poem. Complete that oil painting that's never quite dried.

• Walk away from everything that is not absolutely crucial. Don't beat yourself up for closing the page on that book that didn't hook you. Leave that half-finished basement room the mess it is, and never enter it again. Walk away from acquaintances who promise lunches that you know you'll never share because you really truly won't have time. Ever.

3. Just say yes/Just say no

• Just say yes to a leisurely lunch (hold the guilt sauce) with someone you really care about, even though you're way too busy, and won't have time. Ever.

• Accept an offer of help, even though it's not your style, when you know you need it. 'Nuff said.

• Say yes to staying in your PJs until 6 p.m., and designate a "National Do-Nothing Day" when your mind is bone-tired, your bones won't seem to support you and your soul wants to crawl under the covers and stay there, just for now. Celebrate this holiday with gusto.

• Just say no to requests for assistance -- letters of recommendation or board appointments -- if you're too busy or tired, even though you're flattered and would truly like to help. (If you can't say no straight out, at least take a week to deliberate.)

I realize now that I have learned loads more lessons from cancer than I've listed, and I've only just begun! But I'm going to say yes to something else now -- lunch with myself. And the space for you to share some of the lessons you've learned.

Feel free of course to just say no, let go, or walk away -- whether you've had cancer or not. It's your bedside drawer. Your time. Your life -- which could last a half century. Or just a day. But certainly not forever.

Lisa Belkin: Why A Hospital Should Deny A Transplant To A 'Mentally Retarded' Child

Amelia Rivera is three-years-old. Her mother, Chrissy, describes her as "a happy child whose perfect day involves swinging in the Pooh swing, batting her favorite toy bug under her toy mat, and rolling around on the living room floor to bang her leg on the dog's head. In her eyes, her life is perfect. And after two years of having her in my life, in my eyes her life is perfect."

Amelia was born with Wolf-Hirschhorn Syndrome, caused by a deletion on the fourth chromosome. Its effects include seizures, heart problems, and a shortened life expectancy. About one third of patients die before the age of two, and while some survive into early adulthood, that is relatively rare. Chrissy can not walk, or talk, though "she does recognize us," her mother said in a telephone interview, and "she can communicate her needs. When she rolls across the floor and bangs her legs against ours we know she wants us to pick her up."

In short, it is a heartbreaking disease.

Kidney complications are not the most common part of the Wolf-Hirschhorn checklist, but they are a complication that Amelia has, and doctors at the Children's Hospital of Philadelphia have told her parents, Chrissy and Joe, that the little girl will die in about six months to a year without a kidney transplant.

Doctors also said -- until they changed their minds (somewhat) late Sunday in the face of an Internet outcry -- that they would not perform that transplant.

As Chrissy tells the story, the latest chapter in Amelia's tale began at a meeting at CHOP last week. Chrissy and Joe thought it was a meeting about the steps they needed to take to be tested as potential kidney donors for their daughter, but within minutes they realized that a doctor they had never seen before was telling them that Amelia was not a transplant candidate because she is "mentally retarded."

As Chrissy wrote:

The doctor begins to talk and I listen intently on what he is saying. He has a Peruvian accent and is small, with brown hair, a mustache and is about 65 years old. He gets about four sentences out (I think it is an introduction) and places two sheets of paper on the table. I can't take my eyes off the paper. I am afraid to look over at Joe because I suddenly know where the conversation is headed. In the middle of both papers, he highlighted in pink two phrases. Paper number one has the words, "Mentally Retarded" in cotton candy pink right under Hepatitis C. Paper number two has the phrase, "Brain Damage" in the same pink right under HIV. I remind myself to focus and look back at the doctor. I am still smiling.


He says about three more sentences when something sparks in my brain. First it is hazy, foggy, like I am swimming under water. I actually shake my head a little to clear it. And then my brain focuses on what he just said.

I put my hand up. "Stop talking for a minute. Did you just say that Amelia shouldn't have the transplant done because she is mentally retarded. I am confused. Did you really just say that?"

The tears. Oh, the damn tears. Where did they come from? Niagara Falls. All at once. There was no warning. I couldn't stop them. There were no tissues in conference room so I use my sleeve and my hands and I keep wiping telling myself to stop it.

I point to the paper and he lets me rant a minute. I can't stop pointing to the paper. "This phrase. This word. This is why she can't have the transplant done."

"Yes."

I begin to shake. My whole body trembles and he begins to tell me how she will never be able to get on the waiting list because she is mentally retarded.

In the days after Chrissy wrote that on a blog for Wolf-Hirshhorn families, a petition sprouted online demanding that CHOP change its institutional mind. Amelia now has a Twitter hashtag (#TeamAmelia) and a Facebook campaign full of outrage that this little girl would be denied a transplant solely because she has special needs. The doctors are heartless and discriminatory, those postings say. What about the Hippocratic oath, they ask.

This is not one of those outraged posts.

I do not think Amelia should be but on the waiting list for a transplant.

Kidneys, all human organs, are among the rarest and most precious of the world's resources. The National Kidney Foundation tells us that 104,748 people are currently waiting for a transplant of some kind right now and 4000 names are added to that list each month. Today -- every single day -- 18 people will die while waiting. Each of those patients is someone's child.

The stark reality then, is that a kidney that goes to one patient means it does not go to another. Giving a kidney to Amelia means that someone, whose name you will probably never know, but who will be loved just as fiercely as Amelia is, won't get one in time.

Which is why there are rules -- unemotional, clinical, detached rules -- for a situation that is none of those things. And it is why there are forms like the ones placed in front of Amelia's parents. Amelia is not being denied a donor transplant because she is, as her mother writes, "mentally retarded." She is being denied a donor transplant because she has a cascading syndrome that will shorten and limit her life, meaning that kidney will not "save" her in the way that it might someone who starts out healthier. In cold clinical terms this means that everything it takes to undergo a transplant -- the medications, the repeated biopsy procedures afterwards, the constant monitoring and machinery -- are difficult and sometimes impossible compared with a child who is less impaired. The less mobile a patient is, the far greater the likelihood that she will develop an infection, or pneumonia, or a host of other complications that make it probable that the transplant will eventually fail. Which, in those same cold clinical terms, would make it a waste of an organ.

In a world with unlimited organs it would be reprehensible to deny anyone a transplant because of her limited lifespan, her limited understanding, or her inability to go on to lead a life as full as some other child might. But organs are not unlimited, and there is no way around these choices. It is a most imperfect and wrenching system. It is inexact and capricious. It brings me to tears as I write about it. But it is the best way we have at the moment.

I was a medical reporter for nearly a decade at the New York Times, and my first book was about the three years I spent observing the inner workings of a hospital ethics committee, so I have seen these decisions made up close. I am also, by way of full disclosure, married to a pediatric cardiologist who has made these decisions as well. It is from that dual vantage point that I write this, and would like to share a few thoughts.

First, I was not in the room when Chrissy and Joe met with the doctors, so I don't know exactly what was said. I do know that too many doctors are awkward or brusque or downright insensitive when delivering difficult news. But I have also had the chance to be in the room when doctors have given this kind of news to patients, and many of those times what the family remembered hearing afterward was not what my notes or my tape recorder heard. I am not sure that there would be any words at all that would have made this an acceptable conversation for the Riveras.

Second, the term "mentally retarded." It makes us cringe. It felt to Chrissy and Joe like a slap, a diminishment, a denigration. But in the language of medicine it is simply a descriptive term, one used in its clinical sense and one that fills the medical literature and is used every day. There's a separate but worthwhile debate to be had over whether it is time for the profession to find a new term -- ("I know it's just a word," Chrissy said when we talked, "they should find another word") but it was used on that form as a clinical diagnosis, not an aspersion or a dismissal.

Finally, for perspective and advice I called Dr. Kurt Hirschhorn, a pediatrician and geneticist at The Mount Sinai School of Medicine, and one of the two researchers who identified Wolf-Hirschhorn Syndrome in the early 1960s. He was also the head of the Mount Sinai Hospital Ethics Committee for 30 years (and, as it happens, he"s been a mentor to my husband at Sinai, and we consider he and his wife our friends.) Kurt read Chrissy's story and I asked him, "Should Amelia be put on the donor list for a kidney transplant?:

No, he said.

"What our ethics committee would tell the parents is that we can't give her a kidney, because there is a shortage of kidneys, especially for little kids, and her impairments are too significant," he continued.

Throughout this article I have been careful to use the term "donor kidney," meaning one from an anonymous deceased donor, per the protocols of the national organ registry. There is another kind of kidney donation, however -- from a living designated donor (you can live with just one kidney) but Chrissy says the the hospital rejected that option during last week's meeting. Dr. Hirschhorn said he did not understand why. As an ethicist he would want to make sure that a potential living donor fully understood the personal risks of such a donation, and also understood that Amelia's life would be prolonged, but not saved, by the transplant. If the donor agrees -- ("we either take the risk or she dies" her mother says "so we take that risk") then that might be the best answer to a Solomonic question.

Designated donation is what the Riveras say they wanted to do from the start. "We were never asking that she be listed," Chrissy said, reiterating that they thought last week's meeting was about the procedure for testing potential family donors. "We didn't want her on the list. We don't want to take a kidney from anyone else. We want to give her one ourselves."

Just before I called Sunday night, she said, she'd spoken to the hospital and had gotten word that a transplant for Chrissy could in fact be approved there -- with a living designating donor. The Riveras are relieved, Chrissy said, but also angered by what they have heard these past few days from other families who have been told "your child can not receive a transplant because of cognitive disabilities."

Their growing number of supporters are still angry, too, and posts continue to fill Facebook and #TeamAmelia, raging against a medical system that critics see as treating those with special needs as less worthy of saving.

There is reason for anger here, I agree, but not at the doctors who got into this line of work in order to save lives, and not at the national organ registry that made rules no one really wants to have to make.

The anger should be directed at a system where, of the 12,000 people who died last year and met the criteria to be donors -- 12,000 people whose hearts and lungs and livers and kidneys could have gone on to save many times that number of lives... only half of those potential lifesavers were registered as donors. The rest of the organs went to waste.

So be angry. And then go sign your organ donor card.

Better Survival Rates: Winning hospitals had 17 percent fewer deaths than expected considering patient severity, while non-winning hospitals had 4 percent more deaths than expected.

Fewer Complications: Patients of the winning health systems had 19 percent fewer complications.

Shorter Hospital Stays: Patients treated in the winning system hospitals have a median average length of stay of 4.7 days, nearly half a day shorter than their peers' median of 5.1 days.

Better Patient Safety and Core Measure Adherence: Top health systems had 23 percent fewer adverse patient safety events than expected and had better adherence to core measures of care than their peers.

U.S. health systems with two or more short-term, general, non-federal hospitals; cardiac and orthopedic hospitals; and critical access hospitals were assessed in the study. Researchers looked at eight metrics that gauge clinical quality and efficiency: mortality, medical complications, patient safety, average length of stay, 30-day mortality rate, 30-day readmission rate, adherence to clinical standards of care (evidence-based core measures published by the Centers for Medicare and Medicaid Services), and HCAHPS patient survey score (part of a national initiative sponsored by the United States Department of Health and Human Services to measure the quality of care in hospitals).

Hormones aren’t just for sex—they help control everything from the times when we feel hungry to the timing of our heart beats. Dozens have been described, but there is now a new one on the scene. “There has been a feeling in the field that exercise ‘talks to’ various tissues in the body,” Bruce Spiegelman, a cell biologist at Dana-Farber Cancer Institute and co-author of the new study, said in a prepared statement. “But the question has been, how?”

Speigelman and his colleagues found that exercise—in both mice and humans—starts a cascade of signaling changes, including the production of a never-before-described hormone. They dubbed the new hormone irisin, as a nod to the Greek messenger goddess Iris for its ability to send information to surrounding body tissue.

And the messages irisin carries are not trivial—they seem to effect positive changes in the body. An increase in irisin helps turn white fat into the more beneficial and metabolically active brown fat, which burns more calories. It also seems to make the body more sensitive to glucose, an important capability for keeping diabetes at bay.

In the study, the researchers discovered that exercise increases the body’s production of a metabolism-regulating protein, which in turn stimulates expression of a protein that can produce the new hormone, found to reside in the outer membranes of muscle cells.

The effects of exercise on the hormone’s production seem to be long-lived. Even after 12 hours of rest, mice that had been on a three-week jogging regimen had 65 percent more irisin in their blood than unexercised mice. And people who had gotten 10 weeks of endurance exercise training had double the amount of irisin in their blood than those who had not.

But could this hormone, the scientists wondered, mimic some of the effects of exercise—without subjects having to hit the treadmill? To find out, they injected a batch of obese, pre-diabetic mice that had been fed a high-fat diet with just about as much of an irisin boost as they would get from a workout. After 10 days of injections, the irisin-boosted mice had shed a little weight and become more sensitive to glucose—all without exercise. And a later dissection showed that the hormone spike didn’t seem to have any negative biological effects.

“It is likely that irisin is responsible for at least some of the beneficial effects of exercise on the browning of adipose tissues and increase in energy expenditure,” Speigelman and his colleagues noted in their paper. This find might help explain some of the “afterburn” of extra calories after vigorous activity.

Even if the hormone proves safe for humans to take as a supplement, it won’t replace all the benefits of going to the gym. But it might help people fight obesity and remain more sensitive to glucose, thus fighting off diabetes.

“It’s exciting to find a natural substance connected to exercise that has such clear therapeutic potential,” Pontus Bostrom, a postdoctoral researcher at Dana Farber and co-author on the new paper, said in a prepared statement. The researchers are now also investigating possible effects of the exercise-based hormone on other diseases, including neurodegenerative conditions, and have licensed the finding to Ember Therapeutics (a company co-founded by Spiegelman) for drug development.
