Nuts. There are lots of them. And a lot to know about their nutritional content.

Nuts are healthy, but can also be high in fat. The nutritional values to your health differ wildly between types, and it can depend on whether the nuts are raw or roasted, plain or salted, etc.

So how do you know whether or not you should eat nuts – and which nuts – in order to lose weight or maintain a healthy diet with all the right nutrients?

Here is your cheat sheet – a calories in nuts chart. Print out a few copies, put one on your wall, give it to a loved one, keep a copy in your wallet – you never know when it’ll come in handy. The nuts are listed from lowest to highest calorie.

All of these data came from NutritionData, a great source for nutrition information. And whenever possible, they refer to raw nuts – the importance of this is discussed below. Given the broad range of nutritional content of nuts, the categorization of simply being a “nut” is a woefully inadequate classification. For example, some nuts are low in fat and very starchy, like chestnuts, whereas others are high fat and low carb, like macadamia nuts. Most, however, can be incorporated into a healthy diet.

Lowest & Highest Calorie Nuts – There are so many ways to talk about the nutritional content of nuts, so I decided to tackle this by weight, i.e., per 100 grams. This is about 3.5 ounces or roughly two-thirds of a cup on average – a handful. Keep in mind that for some nuts, this is more of a small meal than a big snack. The amount of calories in 100 grams of nuts ranges from 213 for chestnuts all the way to 718 for macadamia nuts – a difference primarily driven by the fat content. The protein and fat in nuts are what have the potential to help you stick to your diet and feel full if you use them in small portions as snacks.

Nuts With The Most Fiber – Almonds have the most fiber (12% w/w, or 12 g/100g) and cashews & pine nuts the least (3-4%). Chestnuts have the most sugar and Brazil nuts the least. Lastly, peanuts, almonds, and pistachios have the most protein (21-26%) and chestnuts the least (2%).

Nuts Vary By Type Of Fat – Protein quality doesn’t vary widely across different nuts, but fat quality does1 2. In terms of absolute amounts (g/100g), Brazil nuts & macadamia nuts have the most saturated fat, where chestnuts have the least. The presence of saturated fat in nuts is not an unhealthy attribute3; in fact, it’s probably a good thing because polyunsaturated fats are highly susceptible to become damaged when they’re processed, and nuts are frequently pasteurized or roasted, which is capable of causing said damage.

Technical Note: For example, one study showed that standard roasting procedures significantly increased the trans fat content, the amount of peroxidized fats, and advanced glycation end products in a variety of different nuts4. Advanced glycation end products, also known as AGEs, are found at high levels in diabetics and are associated with age-related diseases, Alzheimer’s, cardiovascular disease, and stroke.5 6 7 Further, excess polyunsaturated fats increase the livers susceptibility to injury.

If you can’t find raw nuts or are unsure, stick to those with low polyunsaturated fats like hazel & macadamia nuts, while avoiding those with high polyunsaturated fats like brazil nuts and walnuts. Macadamia nuts are the safest in terms of fat quality. Also, avoid those with the most sugar and carbohydrates like chestnuts and cashews as these can lead to increased AGE production upon roasting.

The essential fatty acid content of nuts (eg, omega-3 & omega-6 fatty acids) should not be a priority in the selection process. If a theoretical optimal ratio of omega-6 to omega-3 fats actually existed, all nuts would be way over it due to their high omega-6 fatty acid content1. Further, in the context of nuts much of the polyunsaturated fat is heat-sensitive and exposed to heat during roasting, and in general, excess polyunsaturated fat increases susceptibility of the liver to damage from a variety of insults.8 9 10 In other words, if you’re having a beer or glass of wine, a bowl of peanuts is not the best choice for a snack.11 12

Low Carb Nuts – If you’re on a low carbohydrate or ketogenic diet, macadamia nuts and pecans are both more than within the acceptable ratio of fats to carbs plus protein (both are >5), however the latter are high in polyunsaturated fats, so check to make sure they’re raw.

Man-boobs

Man boobs might seem like a silly phrase, but it is definitely not silly to anyone who is suffering from excess fat stored in this area. Many men try to alleviate man boobs through spot reduction, or lack of chest exercise, which will both never yield results.

So, how do you get rid of man boobs?

What Are Man Boobs?

For this article, I have created an official definition of man boobs, also known as “moobs”.

Man boobs: (noun) 1. The psychologically distressing distribution of superfluous body fat over a man’s pectoral muscles, creating an appearance similar to breasts 2. Gynecomastia

Each of these definitions has a different look, cause, and treatment.

Man Boobs Type #1: Gynecomastia (Hormonal Imbalance)

Since gynecomastia is more straightforward, I will start with it. Gynecomastia, commonly referred to as gyno, is “the swelling of breast tissue in men or boys caused by an imbalance in estrogen and testosterone.”1 It can occur in one or both breasts, is not uncommon during puberty, and is often an unwanted side effect of anabolic steroids. However, its presence may also indicate a serious underlying medical condition such as cancer, liver disease, or a pituitary tumor.

Excess testosterone is converted to estrogen, which binds to receptors in the breast tissue and causes enlargement. Gynecomastia often manifests as a lump right under the nipple and can be a tell-tale side of steroid use. It is frequently physically painful and can very embarrassing. And though it can resolve spontaneously (especially due to natural hormonal fluxes like those seen in puberty), in older adults it often requires surgical removal.

If you think you may have gynecomastia, I recommend a visit to your health care provider to discuss treatment options and rule out serious potential causes.

Man Boobs Type #2: Moobs (Excess Chest Fat)

The second type of man boobs, or moobs, is much more common and more easily fixed. The typical scenario around moobs is well-exemplified by one of my patients. He is a 32 year-old man, healthy other than carrying around a little extra body fat. At 5’10”, he weighed 210 pounds with 23% body fat by ultrasound. His first words after he sat down in my office were, “I have boobs.”

He told me his chest was too big as it is, and had thus stayed away from intense chest training all together out of fear it would make his chest even bigger. His chest workouts consisted mainly of high rep push-ups in a misguided effort to “tone” his chest and he admitted to doing little in the way of cardio or monitoring his nutrition intake. It was no surprise that this approach yielded none of the desired results, despite strict adherence for 3 years!!!

The distribution of body fat is determined largely by genetics and to a lesser degree by hormones. For example, if the stress hormone cortisol is elevated in your blood, excess calories will be preferentially deposited around the abdomen. Thus, excess chest fat can only be trimmed by following a program designed for overall fat loss, as spot reduction is essentially impossible. Again, it is impossible to target chest fat by doing high rep chest work.

Moreover, many people, like my patient, are afraid to do heavy chest work. This is a huge mistake. I have never met a guy with a six-pack who said his chest was too big. Fat takes up a lot of space, and most people find that after a significant amount of fat loss, their chests look too small, and they regret not focusing more on building the Pecs. I can’t stress this enough: If you have man boobs, it is NOT because your chest muscles are too large. Exercising each (large) body part intensely is key for fat loss, and the chest is no exception.

How Do You Get Rid Of Man Boobs?

1. Exercise

As stated above, you must employ an exercise regimen emphasizing overall fat loss with retention of lean muscle. There are many different specific programs out there, including the Built Lean Program, which are very effective for fat loss. Do not try to spot reduce your chest fat.

2. Nutrition

All of the exercise in the world will not get rid of your man boobs if you are not eating correctly. A maintainable nutrition plan, centered as much as possible around whole, unprocessed foods, and which puts your body in a calorie deficit, will allow the fat to come off. Aim for a fat loss rate of 1-2 pounds per week. Going any faster than that puts you at higher risk for muscle loss and can have longer-term negative metabolic consequences.

Many people are impatient and don’t like that rate, so here is a perspective: My patient’s first visit was in August of 2012. His first goal was to be comfortable taking his shirt off during a Christmas vacation in the Caribbean. He lost fat at a rate of ½-1 pound per week, and after 16 weeks he had lost 14 pounds on the scale and had increased his thigh and arm circumferences. Yet, despite the heavy chest training he was doing, his chest, and waist, measurements decreased significantly.

Over the next 6 months, he lost another 11 or 12 pounds (a rate of about ½ pound per week) and looked great for summer. As the changes he made to reach his goals were subtle and maintainable, he has been able to keep at his target weight and no longer wonders if he should be wearing a bra. So go slow: the future is guaranteed to come. It feels much better to be where you want to be when it does than have rushed to get there and be back to where you started from.

Can You Safely Do Resistance Training When You're Old?

It's no secret that as we age almost everyone loses physical strength. There is a physiological reason for this, but we know that it comes from a shrinking of muscle fibers, called atrophy, or the complete loss of muscle tissue, known as sacropenia. Is this loss a muscle the reason we stop doing resistant training, or is it a self-fulfilling prophecy that since we realize we are getting older and therefore weaker we stop training. And of course we become weaker. Read the full article.

Medicare Announces Plans To Accelerate Linking Doctor Pay To Quality

Medicare Announces Plans To Accelerate Linking Doctor Pay To Quality

Medicare is accelerating plans to peg a portion of doctors’ pay to the quality of their care.

The changes would affect nearly 500,000 physicians working in groups. The federal health law requires large physician groups to start getting bonuses or penalties based on their performance by 2015, with all doctors who take Medicare patients phased into the program by 2017.

The program is a major component of Medicare's effort to shift medicine away from its current payment system, in which doctors are most often paid for each service regardless of their performance. The current system, researchers say, financially encourages doctors to do more procedures and is one of the reasons health costs have escalated. The health law required Medicare to gradually factor in quality into payments for hospitals, nursing homes, physicians and most medical providers.

But the provision regarding doctors is considered the most complicated to enact, and it has encountered strong pushback.

Major physicians groups have been pushing Congress to repeal it, while some health policy experts fear the amount of money Medicare plans to use for incentives will be too small to prod doctors to improve their patient care.

Medicare had already decided that large physician groups -- those with 100 or more doctors, nurses, social workers or other health professionals -- will gain or lose as much as 1 percent of their pay starting in 2015. Those incentives would double to 2 percent the following year under draft regulations Medicare released this month.

The proposal also would phase mid-sized physicians groups—those with between 10 and 99 health professionals—into the program in 2016 instead of in 2017. While they would be eligible for bonuses up to 2 percent, they would be shielded from any penalties for that first year.

In 2017, the program would add the remaining doctors in practices of nine or fewer professionals— about 350,000 doctors, according to Medicare’s estimates.

Pay for many doctors would likely not be altered because their performances were average, although Medicare hasn’t announced exactly how it will dole out bonuses and penalties. There have to be financial losers in the program, because the health law requires that the program not increase the budget deficit.

The bonuses and penalties—formally known as the physician value-based modifier—would be calculated on quality measures that vary from specialty to specialty. Many of those measures check to see how often doctors follow basic medical approaches, such as screening older women with fractures for osteoporosis and documenting in medical records every drug a patient is taking. At least at the start, physicians will be able to select which of Medicare’s measures they want to be judged by.

In determining bonuses and penalties, the government also plans to take into account how much each doctor’s average patient costs Medicare, in order to encourage a more judicious use of testing and more aggressive efforts to avert hospitalizations. Medicare plans to compare physicians against others in their specialties. Physician groups whose patients are the least costly to Medicare—after taking into account their overall health—are eligible for bigger bonuses and softer penalties.

Although Medicare is using the size of all the professionals in a group to determine when they will be phased into the program, the bonuses and penalties would only apply to the bills from physicians.

Medicare said the latest changes were a response to concerns that the penalties were not severe enough to actually change physicians' behavior.

"To really make physicians wake up you're going to have to make it more than 1 or 2 percent," said Michael Kitchell, past president of the Iowa Medical Society and a neurologist at the McFarland Clinic in Ames, one of the state's largest practices. "It isn't that every physician is going to be happy. Some are going to resist these changes. But these quality measures are also evolving so they will become measures that matter."

Kavita Patel, a health expert at Brookings' Engelberg Center for Health Care Reform, said the smaller the medical group, the more challenging it will be for the doctors to adjust to a new system where they must report quality measures to Medicare and may have their pay docked if they underperform.

"Most of these big practices have figured out how to integrate it into their work flow," she said. "If you think of groups of 10 or more, the upside makes a lot of sense, but I’m still concerned about how these smaller practices will too share in downside risk."

The American Medical Association has been urging Congress to eliminate the program when lawmakers complete the annual ritual of adjusting Medicare pay to physicians to avert massive automatic cuts. "To impose a program that takes money off the top of payments that have not kept up with inflation for more than 10 years will increase the migration of physicians into hospital settings, driving up overall Medicare spending in the process," the AMA wrote in a letter to the House Ways and Means Committee in April.

The AMA declined to discuss Medicare’s new proposal.

Protein waffles to be exact…protein waffle recipe thumb Protein Waffle Recipe

It’s been a long time since I’ve made waffles in my waffle maker – throwing a frozen one in the toaster oven is just so easy! But, I wanted a fresh one and threw together a protein waffle recipe from memory. Remember when the blogworld used to make protein pancakes and waffles every Sunday? Oh, the good ol’ days. Luckily, it’s back!

Protein Waffle Recipe

Serves one. Gluten free. Protein full.

Ingredients:

  • 1 scoop protein powder (I used Muscle Milk Banana Crème)
  • 1/4 tsp baking soda
  • 1/4 tsp cinnamon
  • dash salt
  • 1 egg or 1/4 cup egg whites
  • 1 Tb. greek yogurt

Directions: Heat waffle maker and spray thoroughly with oil or non-stick. Mix dry ingredients. Add wet, mix. Pour on waffle maker and cook until done. Don’t check it too often. I let it cook for a good amount of time before I peek, but my waffle maker is old school. IMG 2539 800x533 thumb Protein Waffle Recipe

Top with syrup, peanut butter, sardines, whatever you like.
